Live-In to 24-Hour Care
CommuniCare Nurse Registry refer caregivers to provide care as a live- in; up to 24 hours for your loved one to recuperate and to remain in the comfort of your home; and to have peace of mind in knowing your loved one has the care and support that is needed.
Live-In Care services
The caregiver(s) will live in your loved one’s home, become a trusted caregiver, confidant, and friend. A plan of care will be completed to determine the schedule and services to be performed for your loved one. A live-in care provider is required to have a certain number of hours of uninterrupted sleep while working with your loved one.
It is important to note that Live-In care is not a substitute for 24-hour monitoring. Individuals who require continuous care and monitoring 24-hours a day is not usually good candidates for this type of care, they will likely need multiple care providers to be remained in a home environment.
Live-In Care services can include Personal care, transfer assistance, toileting, personal hygiene, medication assistance, companion, and light housekeeping.

Skilled Nursing
Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) can be referred to provide skilled care ordered by a doctor. The RN referred will conduct an initial evaluation at the start of service to initiate a medical plan of care for the patient, and family. As well as assess additional caregivers needed. Regularly reports to patient’s Physician. The Physician’s medical plan of treatment are carried out by both RN and LPN which may include but not be limited to:

Home management assistance is available by CommuniCare Nurse Registry include Housekeeping, Laundry, Cleaning Refrigerators, Minor Home Repairs, Sewing, Transportation, Meal Planning and Preparation, Shopping Assistance, etc., these duties are routinely performed by Homemaker or Companion trained caregivers. If the client health condition requires the caregiver assessment of skills are necessary to determine a client health status may change, and hand on care require the services of a CNA or HHA.
